Linker "Remove Link" button doesn't work

Description

Clicking the "Remove Link" button fails at the following line because this.form is null: 

 this.form['[href]'].value = "";

I verified this on the latest nightly example on Xinha.org.

Looking into this further, it seems there are some other places where Linker was never updated for the new dialog system (e.g., switching radio buttons for target type causes a JavaScript? error). I'll start at fixing this shortly.

Changeset r1097 fixes these issues (switching target types causes a JS error and remove link doesn't work).
